HealthPathways

Developed by local GPs, specialists, nurses and allied health providers, HealthPathways supports health professionals by providing local, relevant information on managing medical conditions and referral options for their patients. 

HealthPathways is a web-based information portal providing access to over 700 local clinical and referral pathways for a wide range of conditions. 

It is like a ‘care map’, so that all members of a health care team – whether they work in a hospital or the community – can be on the same page when it comes to looking after a particular person.

Western Sydney HealthPathways is a partnership initiative between WentWest, the Western Sydney Primary Health Network, Western Sydney Local Health District, and the Sydney Children’s Hospitals Network. General practitioners, hospital specialists, and allied health providers are actively involved in its creation and continuous review, ensuring its safety, relevance and effectiveness.

A team of clinical editors are working on new pathways in addition to continually reviewing and updating pathways to reflect recent changes to clinical guidelines.
